Barnsley Choices have had some enquiries from providers with regard to the Day Care application packs and especially with regard to changes with the forms that each group daycare provider needs to complete for each person living and working on the premises. These forms are in place in order for Ofsted to carry out all relevant checks including with social services so that each individual is considered to be suitable to be in contact with children. Below is a brief outline of all the relevant forms that providers need to be aware as this can affect their registration.

DC2	* All members of staff except person in day-to-day charge * The applicant including committee members, partners, directors or nominated unless person in day-to-day charge * Any person over aged 16 or over living or working on the premises * Owner unless they are person in day-day charge, they should complete DC2A instead.	Will depend on individual setting and whether they have completed a DC2 in the last 3 years
DC2A	The person in day-day charge	One
Health Declaration Booklet	* The person in day-day charge * The Registered person if also in charge or in contact with children	Depends on individual setting
DC3 Notification to Ofsted	To notify of any changes to staff /people on premises after original application NB these people may also need to complete DC2 or DC2A	Depends on individual settings
